FORMS > Nomination Form South Carolina TRiO Nomination Form (*) Required Fields WHBSLC Leadership Award Information The WHBSLC committee will award a 2011-2012 Outstanding Leadership Award for two dynamic student leaders (1 Pre-college & 1 College/Adult Learner) who attend this year's conference. Students are nominated by their Program Director (see Nomination Form) and will be recognized for their outstanding leadership qualities during the WHBSLC Banquet on Saturday, January 14, 2012. Who should you nominate? This is an opportunity for Program Personnel to recognize the best of the best. Nominate those students who demonstrate outstanding leadership qualities, maintain a positive attitude, serve as a role model for his/her peers, is always respectful to others, exemplify and illustrate the significance of the WHBSLC. Each program should limit their nomination to one student. This student is required to attend the conference. How do I nominate a student? Complete the TRiO Leadership Award nomination form below Include one nomination letter and one letter of support: The nomination letter should come from TRIO Personnel or Advisors for the 2011-2012 year. The letter of support should come from someone in the community (i.e., teacher, school personnel, minister, etc.). Include student's resume or activity sheet (include those things that make the student stand out above the rest, i.e., community service, initiative, newspaper articles, etc.). Complete the WHBSLC Leadership Award nomination form by December 2, 2011. Please send the completed forms (mentioned in steps 1-3 above) by email or fax only (see contact information below).
